2010 Communities For a Lifetime Award Entry Form

Entry Guidelines for the 2010 Communities for a Lifetime Awards

Eligibility for Participation

  • Communities that have adopted a resolution or proclamation prior to December 1, 2009

Communities (cities, towns, counties and villages) that have adopted a proclamation or resolution in support of the Communities for a Lifetime initiative and that have remained active in the process.  Communities that are new to the process are eligible if their community adopted its resolution or proclamation prior to December 31, 2009.

How to Apply

Only actively participating communities will be considered and will have the opportunity to submit and complete the online entry form detailing information about their community’s initiative from January 2009 to December 2009.  Once your information is completed and submitted, you will not be able to save, edit or add additional information. During the online application process, you will not be able to save portions of your information.  The entire form must be completed once the entry process begins.  The process will not allow you to save for completion at a later date. 

Please thoroughly complete the contact information, so that we may contact you in the event there are questions regarding the information submitted.  The person designated by your local government as the Communities for a Lifetime coordinator is the preferred contact person.

Entry Deadline

All information must be submitted by Friday, April 16, 2010, at 5:00 p.m. (Eastern Daylight Savings Time).

Notification of Finalists

Communities from both levels of eligibility will be chosen from around Florida for their exemplary initiatives, programs, services and best practices for the 2010 Communities for a Lifetime Awards. 

Awards Ceremony

All honorees will be recognized on Friday, July 9, 2010, in Tallahassee, Florida.
